<br />IIV-A(Cont.) I COMPONENT NARRATIVEjCONTINUED (attach for each component) <br />(31 Describe the snecific referral, screening, admission and terminationorocedures. <br />Admission is by court referral through the Juvenile Justice and Prevention Program. Each client will be screened by <br />a licensed therapist or counselor using a variety of screening tools, including the J-SOAP-II, DSM-IVand/or <br />Adolescent SASSI. Also, Psycho-Educational testing will be completed using standardized industry testing tools. <br />Family Dynamics Assessment will be administerd to client and family members. If individuals meet the criteria for <br />treatment, a treatment team will determine the level of services required for the individual and the family members. <br />Clients will remain in the ALPHA program while they are involved with the court system, or until all parties agree that <br />the youth has addressed the problem behaviors adequately and has satisfied all court requirements. If it is <br />determined that further treatment is needed after probation has been satisfied, the client may continue with the <br />program. <br /> <br />Parents may terminate the child's treatment when proper reporting has been made to the court officials. <br /> <br />Termination may also occur during the program if the client is non-compliant with the program's policies and procedur <br /> <br />1(4) Describe imn/ementation to include: <br />a) The primary and supplemental services provided, if applicable: <br />Primary services will be outlined in the Individual Plan of Action for each client and will consist of individual and group <br />counseling sessions. Supplemental services will be made available for family therapy as outlined in the Individual <br />Plan of Action and will consist of family counseling with emphasis on education, support and group therapy sessions. <br /> <br />b) The means of working with youth and families: <br />The Youth will be povided with individual andlor group counseling as indicated in the Plan of Action for the client and <br />his or her family. Sex offender specific assessments, as well as other assessment tools for issues such as substance <br />abuse, anger management, and life skill development will be administered by certified therapists. The Juvenile Sex <br />Offender Assessment Protocol-II will identify areas of concern which must be addressed by client. Family members <br />will also have assessments to help determine family functioning and identify areas of concern and problem behavior. <br />Family members will be provided with individual counseling andlor group therapy sessions. They will attend family <br />night meetings which mayor may not include the Youth. <br /> <br />c) The daily activities: <br />Each client and family member will receive services as outlined in his or her own Individualized Plan of Action. The <br />Youth will attend weekly group sessions, (unless otherwise stated), and attend individual counseling as outlined in the <br />Plan of Action and treatment plan is for a minimum of 9 to 12 months. Family members will attend specified group, <br />individual, and family night sessions. Specific client andlor family issues will be addressed during individual or group <br />counseling sessions. All groups will incorporate cognitive-behavior training, Reality and Experiential models of <br />therapy, Psycho-Education, and will include such topics as: Life Skills, Communication Skills, Social Skills, Problem- <br />Solving & Decision Making Skills, Anger Management, Peer Presure, Family Issues, Family Interaction, Impulse <br />Control, Substance Abuse Issues, Cognitive Process, Trigger Identification, Relapse Prevention, and <br />Preventionllntervention. <br /> <br />F=-15 <br /> <br />Page 5 of 10 <br />
