If services are delivered at <br />multiple locations, please include information for all locations. <br />The clients will receive setvices at Genesis...A New Beginning located at 17 Cabarrus Avenue West, Concord, N.C. 28025. <br />The facility consists of offices for each therapist and rooms for group and individual counseling. This office is conveniently <br />located across the street from the city parking deck. <br /> <br />(2) Identify paid or volunteer staff qualifications, (certifications, degrees, work experience) and their responsibilities relative to <br />this component. <br /> <br />1. Michael McCloskey: BA, MA-Counseling, LPC; Experience in counseling field since 1987 with experience in juvenile <br />and family counseling, troubled adolescent counseling, substance abuse counseling, domestic violence counseling; <br />currently involved with Sexual Offender Youth Counseling <br />2. Shirley Dennis: AA Health Setvices; BA Psychology; CSAC; 10 years experience in substance abuse counseling, <br />sexual offender youth and adult counseling, domestic violence counseling <br />3. Donna Wise: BA Post graduate work in Pastoral Counseling: 6 years experience in Pastoral Counseling, Small Group <br />Counseling; Christian Seminar Planning, Speaking, Facilitating; <br />4. Barbara Bell, BA Sociology, MA Counseling, 10 years+ experience with mental health counseling <br /> <br /> <br />(3) Describe the soecific referral, screening, admission and termination procedures. <br /> <br />Admission is by court referral through the Juvenile Justice and Prevention Program. Each client will be screened by a licensed <br />therapist or counselor using a variety of screening tools, including the DSM-IV , Adolescent SASSI. Rosenberg Risk <br />Assessment, Debereux Scale of Mental Disorders, andlor Multiphasic Sex Invenotry II. Also if required, Psycho-Educational <br />testing will be completed using standardized industry testing tools. If the person meets the criteria for treatment, a treatment <br />team will determine the level of services required for the individual and the family. <br /> <br />Clients will remain in the ALPHA program while they are involved with the court system, or until all parties agree that the youth <br />has addressed the problem behaviors adequately and has satisfied all court requirements. If it is determined that further <br />treatment is needed after ptobation has been satisfied, the client may continue with the program. <br /> <br />Parents may terminate the child's treatment when proper reporting has been made to the court officials. <br /> <br />, <br /> <br />Termination may also occur during the program if the client is non-compliant with the program's policies and procedures as <br />addressed in the clienfs Individual Plan of Action.
