<br />Primary services will be for those outlined in the Individual Plan of Action for each client. Supplemental services will be made <br />available for family therapy as outlined in the Individual Plan of Action <br /> <br /> <br />(4) Describe imnlementation to include: <br />a) The primary and supplemental services provided. if applicable: <br /> <br />b) The means of working with youth and families: <br /> <br />The youth will be provided with individual andlor group counseling as indicated in the Plan of Action for the client and his or her <br />family. Assessments for issues such as substance abuse, sexual offense, anger management and life skill development will be <br />administered by certified therapists. Parenting classes are available if deemed necessary. <br /> <br />c) The daily activities: <br /> <br />Each Client will receive services as outlined in his or her own Plan of Action. The youth will attend weekly (unless otherwise <br />stated) groups session for a minimum of 12 months. All groups will incorporate cognitive-behavioral training and will include <br />such topics as : <br />. <br /> <br />Specific client issues will be addressed during individual or group counseling. <br /> <br />(5) Describe the interaction with juvenile courland other referring agencies. <br /> <br />ALPHA staff will attend juvenile court when in session, meet with court counselors to review cases and provide ongoing <br />consultation as needed. <br />Other agencies will have a contact person in the program <br /> <br />~-4- <br />
