<br />III Serv1ce Program Description <br /> <br />Specified below is the service or services to be provided by the Vendor under <br />this agreement. Service{s) are specified in ter.ms of the service definitions <br />authorized in North Carolina's Comprehensive Annual Services Plan pursuant to <br />Title xx. <br /> <br />1. The Adult Day Care/Day Health Program will comply with standards for <br />certification revised 5/1/92 as printed. <br /> <br />2. The Adult Day Care/Day Health Program will provide for the following <br />services: <br /> <br />A. An .organized schedule of activities which are based on dements of <br />the participants' individual service plans consistent with stated <br />program goals and will allow for group and individual activities. <br />Activities will include a balance of the following five types of <br />activities; diversional,educational, social, volunteer service and <br />program assistance. Activities will be planned jointly by staff and <br />participants. <br /> <br />B.Referral to and assistance in using appropriate community <br />resources, including arranging for and, if necessary, providing <br />travel. Appropriate community resources include but are not limited <br />to medical facilities, service agencies, businesses, shopping <br />facilities, etc.; <br /> <br />C. A nutritious noon meal which provides at least 1/3 of an adult's <br />daily nutritional requirement and nutritious morning and afternoon <br />snacks. Any special diet prescribed in writing by a physician will <br />be provided; <br /> <br />D. Transportation, defined as carrying participants to and from <br />their homes, provided by center vehicles. Transportation provided <br />by other means, i.e., family members, EHTAP, will not be reimbursed <br />by.Adult Day Care funds. <br /> <br />3. The day care/day health program will be open five days each week, except <br />for designated holidays and hazardous weather conditions, in which ease to <br />local Cabarrus County public school policy will be followed. The Center <br />maybe closed four to five staff work days per year to. provide time for <br />staff conferences, planning and evaluation of service plans and training <br />sessions. The day care program will notify the Department of Social <br />Services of such dates within the first week of the month via monthly <br />calendar. <br /> <br />4. The Department of Social Services will provide supportive services and <br />consultation to the day care/day health program regarding participants <br />certified by the department and regarding program operation. Supportive <br />services regarding participants include: assistance in developing and <br />reviewing service plans, scheduled consultation regarding participant <br />needs and concerns, evaluation andplanninq with the participant and <br />program staff at least annually regarding the effectiveness of and ongoing <br />need for adult day care, and supportive services regarding certification <br />standards. <br /> <br />~b <br />
