Using the successful <br />model perfected in six years, our goal is to offer 4-H PACK (Positive Activities for Cool <br />Kids), a theme-based summer day camp, with other supplemental grant funding. <br /> <br /> Collaboration with the school and other community resources will help provide <br />effective and efficient services to our children. Through the services provided and the <br />recruitment of volunteers from the community and collaborative agencies and school <br />staff to serve as positive role models and as tutors, a reduction in problem beh_aviors <br />and academic failure of the students should result. <br /> <br /> Evaluation strategies used to measure outcomes will inclL~de (1) parent and <br /> student assessment surveys; (2) teacher and staff assessment to track the student's <br /> grade improvement; (3) attendance rates, (4) in-school and out-of-school suspension <br /> rates; (5) parent, student, and staff mid and year-end evaluation of student progress <br /> and the program, and (6) participation with EdStar SOS evaluation data. <br /> <br />B. Pro.qram PlanninR & Desi.cln <br /> <br /> The results of periodic assessments and evaluations are used as students help <br /> <br /> plan snacks, social, enrichment and recreational activities, service learning projects, <br /> and Fun Friday activities. Students help design the rules for responsible behavior and <br /> the consequences of infractions as a behavior management technique. <br /> <br /> The Advisory Board members are very supportive and willingly share practical <br /> ideas for the program. Additional school staff including school counselors and Wes <br /> Seamon, Chief Juvenile Court Counselor attend meetings and provide insight.
