III. PILOT PROJECT DESCRIPTION <br /> <br />1. Experience <br /> <br />As indicated earlier, our department has been the CAP/DA lead agency since 1988 utilizing <br />Medicaid as the primary payer for services provided. We are actively serving 187 persons with <br />the capability to serve 256 persons at any one time. Since August of 2002, we have served 231 <br />disabled and/or aged persons. We successfully managed an in-house aide program for 14 years. <br />Through CAP/DA we are presently serving a diverse citizenry. The two largest cities in the <br />metropolitan area are Concord and Kannapolis. They, therefore serve the most consumers. <br />However, we have reached every city and town in the county. Two of our clients have Charlotte <br />addresses, but reside in Cabarrus County. We have always had a good working relationship <br />with all of our providers. Primarily we use two suppliers to provide supplies and equipment to <br />our customers. However, we have many other suppliers serving our program based in and out of <br />the county. <br /> <br />For over 10 years we have used the concept of having one to two CAP/DA case managers, <br />depending on applicant census, do outreach and prescreen potential participants. This has given <br />us the opportunity to have two skill sets. One specialty lies in putting on new persons quickly <br />and efficiently. The other allows for a deeper concentration in ongoing case management to <br />assure client needs are addressed promptly and all services monitored very closely for quality <br />assurance. <br /> <br />The in-home aide (IHA) supervisors work well together to assure that the clients are regularly <br />staffed. There are two IHA Supervisors on staff and one RN supervisor. Both of the IHA <br />supervisors have been with the program for over five years. The RN supervisor has been with <br />the agency for three years and also completes the physical health portion of the CAP/DA <br />assessments thus giving her intimate knowledge of the tasks needed by our clients.
