Sponsoring anency, Department of Social Services. <br /> <br />Priorities Addressed. <br />List the priority (and other) risk factors or d~spos~tional option(s) addressed by this program: <br /> <br />Home Based Services: 1) Family Management problems: One of the primary objectives of Home Based Services is to help <br />families build structure and consistency within famihes. Home Based services work with families to create appropriate <br />and meaningful consequences, set boundaries, improve disciphne methods and enhancing communication and anger <br />management skills. In turn, these actions help to create stucture in the home and may also help in the prevention of 2) <br />substance abuse. If a parent has a substance abuse problem, appropriate referrals are made to community resources. <br /> <br /> Guided Growth program: 1) Fan~ly Management Problems: the group participants develop an understanding of their role <br /> ~n the fmxuly and thmr abd~ty to ~nfluence the success of the family. They learn skills that enable them to function more <br /> successfully ~n the home environment; such as, how to problem solve and compromise. 2) Drug and alcohol abuse: they <br /> learn how this can underrmne achievement and success in life. They also learn how to get help with substance abuse <br />. problems <br /> <br />Program Description. <br />Provide a brief description of the program to be implemented in the county: <br /> <br />Home Based Services: seeks to empower families to cope with stressors, establish appropriate limits on behavior, provide <br />necessary support and superwsion, and identify values that will help promote success. Children involved in the program <br />are between the ages of 10-17 and must be referred by the Depatment of Juvenile Justice.The workers meet with the <br />farmlies weekly and can be reached 24-hours a day by pager. <br /> <br />Graded Growth' The group provides a a stuctured curriculum ~ncluding top~cs such as anger management, communication, <br />problem-solving, goal development, and ~ndependenet hving skills.
