and will utilize the JobLink Center for much of the services provided. A part of what the <br />PREVENTS worker does will be in the capacity of a guide or coordinator of services. A <br />primary problem of many customers who fall in the category targeted by PREVENTS is a <br />lack of motivation resulting in a lack of follow-through in making sure that they do <br />everything necessary to "land the job". The PREVENTS worker will not only provide direct <br />services to the customer, but will also use the JobLink Center for group counseling, short- <br />term pre-job skills to aid in job obtaining and retention, as well as working with other <br />Partners at the JobLink Center to make sure that the customer secures their services as <br />well. <br /> <br />Describe how comprehensive and specialized assessment of the skill levels and service needs of <br />Adults/Dislocated Workers, including diagnostic testing,, use Of other assessment too/s, in-depth <br />interviewing, and evaluation to identify employment barriers and appropriate employment goals, <br />will be accomplished at the JobLink Career Center. List assessment resources to be used, <br />including se/f-administered and those requiring staff assistance. The PREVENTS worker will <br />ensure that the customer avails himself of any appropriate assessments at the JobLink <br />Center. Based on the PREVENTS workers comprehensive assessment of thi~ customer, it <br />will be determined at what level they are functioning, and what tools at JobLink will be <br />needed f(~r further testing. Even though much of the testing is self-administered, the <br />PREVENTS worker will closely monitor the customer to ensure complete follow-through of <br />the test. ' ' <br /> <br />o <br /> <br />Describe how an individual employment plan will be developed that identifies employment goals, <br />appropriate achievement objectives, and the appropriate combination of services for the <br />participant to achieve the employment goals. Describe how the plan will be reviewed and <br />UPdated. Following all assessments, the PREVENTS worker will closely review all results, <br />and at that' point develop an individualized employment plan with the customer. It will <br />identify goalS, steps to goals that are attainable by the customer, as well as supportive <br />services arranged by the PREVENTS worker to aid the customer in meeting this goal. <br /> <br />Describe planned case management services. Describe how and when counseling will be <br />provided to Adult/Dislocated Worker parti~ipants at the JobLink Career Center. Describe how the <br />WIA participant's progress and attendance will be monitored. Depending on the abilities and/or <br />motivation of the customer, case management Services will be provided as often and at <br />levels determined necessary to meet the goals. These can be provided at the JobLink <br />Career Center with partner staff or with the customer and PREVENTS worker on an <br />individual basis in the community with potential service providers and employers. <br /> <br />Describe how the need for short-term pre.vocational services for Adults/Dislocated Workers wi//be <br />determined. Describe how those short-term prevocational services will be delivered to <br />Adults/Dislocated Workers. The need for short-term prevocational services will be <br />determined at both the PREVENTS assessment as well as at the scheduled assessments <br />at the JobLink Center. Provision of services will depend on services needed. Some can <br />be provided by the PREVENTS worker and some will be coordinated with other providers. <br /> <br />D. TRAINING SERVICES <br /> <br />Describe how eligibility for training services for Adults/Dislocated Workers will be determined at <br />the JobLink Career Center. When a customer' has been determined eligible for the Intensive <br />Services, and has been .assessed and served by the PREVENTS worker with assistance <br />from the JobLink Career Cente~r, and they are still unable to secure or maintain a job, they <br />would need to receive training services. The services will be based on appropriateness, <br />need, individual skill level (ability to complete), area of interest, and availability of <br />employment in that area. <br /> <br />2.
