Aging Programs Coordinator <br />(Information & Case Assistance / In-Home Service Coordinator) <br /> <br />General Definition and Conditions of Work: <br /> <br />Responsible for establishing and maintaining the database for all Aging related services and for all Aging <br />Services clients in Cabarrus County. This position serves as the liaison between the Cabarrus County <br />Depa,haent of Aging, the Centralina Area Agency on Aging, and the N.C. Division of Aging Information and <br />Assistance programs. Performs administrative work supervising the In-Home Services program. Work is <br />performed under the regular supervision of the DireCtor of Aging Services. <br /> <br />Essential Functions/Typical Tasks: <br /> <br />Coordinate the Information and Referral program for ali Aging related services in Cabarrus County; <br />Performs intake-verification-processing of client information. Coordinate client needs assessments and <br />makes referrals with local Aging service provider agencies/organizations; <br />Develops electronic and manual forms, applications and other information tools for use in documenting <br />Aging related services in Cabarrus County; <br />Coordinates and serves as an ex-officio member supporting the activities of the Cabarrus County Department <br />of Aging Information and Assistance Steering Committee. Plays a key role in administering the policies <br />and procedures; <br />Serve as the liaison between the Cabarrus County Department of Aging and all other local Aging service <br />providers; <br />Serve as the Cabarrus County Department of Aging's representative and liaison to all local, regional and state <br />Disaster Relief and/or Emergency Management committees; <br />Coordinates the In-Home Services program; maintains records and files, supervises the direct care of clients; <br />Maintains a case load of In-Home service clients. Proves counseling, needs assessments and referrals; <br />Supervises and evaluates the In-Home Services staff; <br />Perform related tasks as required. <br /> <br />Knowledge, Skills and Abilities: <br /> <br />Must be able to work independently with a diverse population of older adults, general public and service <br />providers. Must be able to administer the In-Home Services program and supervise subordinate staff. <br /> <br />Education and Experience: <br /> <br />Requires bachelor degree in Social Work or related Human Services / Gerontology field and two years <br />experience in the Human Services field - preferably with Aging related services; must be computer literate <br />with knowledge of Work 97 / Excel and be able to learn and train local staff on Iris Information and Referral <br />software (training and technical support provided); knowledge of Interact I E-mail preferred; or equivalent <br />combination of education and experience. Must have reliable means of transportation. <br /> <br /> <br />
