I~PAIR / MODIFICATION PROCESS <br /> <br />Inspections <br /> <br />I. After application and verification of income to insure that individual qualifies the <br /> Program ManagerAVeatherization Coordinator will visit the client's home to inspect <br /> and complete an assessment of the clients repair/modification needs. He will ask the <br /> client what their needs are and explain what can or can not be repaired under the <br /> Urgent Repair Program. <br /> <br />2. Work Write-ups - Program Manager/Weatherization Coordinator will complete a <br /> work write-up of all repairs/modifications. <br /> <br />Contractor Procurement - Some work shall be performed by recipient agency as the <br />general contractor where this method is used. The Weatherization Coordinator shall <br />complete a cost estimate of all materials and labor to complete the work. He shall to <br />the maximum extent use the proper procurement process to purchase materials and <br />supplies. Some work may be done by recipient agency as the General Contractor and <br />part of the work sub-contracted by licensed sub-contractors. <br /> <br />Work performed by other contractors when work is contracted to a General <br />contractor. Weatherization Coordinator will complete a work write-up to spell out <br />materials to be used and the work process. These work write-ups and specifications <br />shall be mailed to contractors wishing to bid on the work. These work write-ups and <br />bid invitations shall include the clients' name, address and phone number. The work <br />write-up bid invitations shall state time and deadline. Client must allow these <br />contractors access to their homes so the contractor may inspect and make <br />measurements to bid on the work. Contractors shall carry proper insurance. <br />Homeowner must carry homeowners insurance. <br /> <br />Contracts - Contract forms shall be completed by Weatherization Coordinator for each <br />client's home whether work is done in house by recipient as the general contractor or <br />work is performed by an outside general contractor. <br /> <br />Change Orders - All change orders must be in writing and signed by the Agency, <br />Contractor and home owner, prior to any work being performed that was not spelled <br />out in the original contract. <br /> <br />Pre Construction Conference - Weatherization Coordinator shall set a date, time and <br />location to meet with the home owner, general contractor, to go over work write-up, <br />specifications and contract. A pre-construction conference record sheet shall be <br />completed and signed and dated by representatives of the agency, contractor and <br />home owner. They shall agree on a date for work to begin. The contract shall be <br />signed at this time by all parties involved. <br /> <br />Page 4 <br /> <br /> <br />
