SECTION [FI: PAYMENT FOR SERVICES <br /> <br /> A. In consideration for the performance of services set forth in Section I above, the <br />Provider shall receive an hourly rate of$11.00 per client treatment hour for IOPT, KOPT, <br />and Aftercare. Assessments will be billed at the rate of $25.00 per assessment. For billing <br />-ptrrposes, a client treatment hour must equal a minimum of forty-five minutes of service <br />provision. <br /> <br /> B. Payment for services are to be paid monthly and not to exceed $10,000 and not <br />to exceed $127,000 yearly. It is understood that this contract is based on 15 months <br />~cherefore, the ceiling amount of $10,000 per month will be in effect for the first quarter of <br />operation. If in any month, the billing hours exceed $10,000 the Provider agrees to <br />continue providing services to all new and current clients. The service hours not <br />compensated will be held until such time when money is available as a result of unused <br />-funds from a lower billing month. If this does not occur, the Provider agrees to donate <br />those service hours to the program. <br /> <br />C. Provider shall submit to the NC Division of Community Corrections named <br />-contact in Cabarrus County monthly invoices for actual treatment services provided to <br />said clients during the preceding thirty days. Invoices shall be submitted within the first <br />five working days of the next month for the actual number oflOPT and ROPT treatment <br />hours. It is a~oreed that the Provider will not cha~e for canceled appointments or for <br />~a~ture by clients to show nor will the Provider charge a "drop out" penaky for a client <br />who discontinues treatment prior to completing all clinical services specified in the <br />individualized treatment plans. <br /> <br /> D. Each monthly re~tuest shall include the number of'hours provided according to <br />-type/phase, the number of individual sessions, the number of assessments conducted and <br />the total number of clients served by phase. Addendum B <br /> <br />SECTION IV: REPORTING AND COMPLIANCE <br /> <br />A. Provider shall provide the appropriate representative to attend all client staffings, <br />court sessions as necessary, senior advisory committee meetings, conferences and <br />training seminars. <br />-A. Provider shall be required to provide on a monthly basis, client specific <br /> information to be utilized by the Court to monitor treatment compliance and <br /> produce reliable outcome measures.
