PART Ill (of 3 Parts) <br /> <br />A.) Weatherization Assistance Program. The following shall apply to the Weathedzaticn <br />Assistance Program as no[ed in Part I, Section 4At <br /> <br /> Secticn 1: procjram Guidance. The Subgrantee shall provide to the Subcontractor <br />copies of ail rules and regulations, instructions, and other documents pertaining to [he <br />Program. in no instance shall the Subcontractor take action on any rule, regulation, or <br />instructive printed in the Federal Register or other document, without ~rs~ notifying the <br />Subgrantee cf such intention. In the event that rules, regulations, or p?ocedures are revised <br />or amended, the Subgrantee will forward such revisions or amendments to the <br />Subcontractor and thereby make them a part of this Agreement. <br /> <br /> Section 2: Certification _of Ap01icants. The Subcontractor shall give priority to <br />identifying and providing assistance under this Program to eiderly and handicapped <br />Iow-income persons and to provide assistance to those persons whose income is at or below <br />150% of the poverty level as determined by the Oh'ice of Management and Budget, or who <br />received cash assistance payments under Title IV or XVI of the Social Secudty Act or other <br />applicable State or local law. Appt/cants must have received assistance during the <br />preceding 12 months in which weatherJzatJon funds are being appropriated. The <br />Subcontractor must certify the eligibility of each household by completing the DOE <br />Homeowner Certification Form, EIA-29D. Eligibility shall be certified no less than six (6) <br />months prior to receipt of weatherization assistance. In those instances where dwellings are <br />not owned by the applicant, the Subcontractor shall execute an Agreement for Participation <br />for Leased Dwellings. The Subcontractor must also obtain a Release Work Order from the <br />applicant prior to the actual weatherization activities. <br /> <br /> Section 3; Prioritization ¢./.f Client Categories. The Subcontractor shall give priority to <br />identifying and providing assistance under this Program to elderly and handicapped <br />low-income persons. The Grantee has established storewide completion goals of 39% <br />e!derly, 14% handicapped and 47% other. In addition, the Grantee has established a <br />storewide completion goal of 31% rental units. The Subcontractor shall, to the maximum <br />extent practicab/e, attempt to meet these percent:age goals. <br /> <br /> Section 4: The Subcontractor shall not exceed an average cost of $ per <br />dwe!ting unit completed (not inclu.di~g administrative costs) nor shatl it exceed $'~,2fO <br />materials cost on any individual unit completed. Where it is determined thai the average unit <br />cost cr the individual unit materials cost is insufi']cient to meet c~ient needs, the <br />Subcontr.~ctor shall submit a written request for a Materials Cost Waiver to the Subgrantee <br />who will in return request a waiver from the Grantee for approval. <br /> <br /> Secticn 5: Schedule of Imclementation. The Subcontractor acrees that a minimum <br />cf dweJiing units are to be weatherized during the grant period defTned in Pan I, Sect/on <br />3. in addition, the Subcontractor agrees that if the rate of completion of weatherized units <br />fails to a level that wiil prevent the Subcontractor from utilizing the full amount of funds <br />.crovided under this contract, then the Subcontrac,'or w/il notifl/ [he Subgrantee of such a <br />situation and allow the renegcdstion of the contract to a funding level consistent ,,vith the <br />Subccn~rac~or's current rate o~ production, fn the eve,qt that the Subgrantee receives an <br /> <br /> <br />
