The Department shall retain all budgets, budget revisions, <br /> cDntr~cts, contract addenda, and financial records in accordance <br /> with the current Records Disposition Schedule for county and <br /> district health departments issued by the Division of Archives and <br /> Bistory, Department of Cultural Resources. <br /> <br />2. The Department shall prepare and maintain a budget for each activity <br /> covered by this contract in a manner consistent with instructions <br /> providex] with DHS Form 3370 (Rev. 3/86) and DHS Form 2948 (Rev.2~87). <br /> <br /> a. The Deparhnent shall prepare budget revisions for prior approval of <br /> the State when ~J]ose revisions are Jn the School Health Program <br /> (Line It~ 6200) or DeliveIy Sel-~ices Program (Line Item 6869). <br /> <br /> b. The Depar~-ment shall prepare budget;revisions fc.r prior approval of <br /> ~J~e State when State funds will be. increased or decreased. <br /> <br /> c. ]~e Department shall prepare an informational copy for the State of <br /> all other b~w]get revisions when proposed expenditures exceed the <br /> line amount budgeted. <br /> <br /> d. The Department shall su~ait all revisions prior to the end of the <br /> tern specified in ~J~is Contract. Budget revisions received by the <br /> State after the end of the contract period v~ll be returned without <br /> action. <br /> <br />3. The Department shall observe the following conditions when budgeting and <br /> re~rting earned inccxoe revenues: <br /> <br /> a. All earned income, must be budgeted in the program ~ere earned, <br /> e~.c~:pt that inccr0e earned by a progr~m~ which has no activity budget <br /> can be budgeted in a program approved by the Division. <br /> <br /> <br />
