The Depat-hment shall complete HHS Foim 441, Assurance of Cc~plianCe with the Department of Health and I~elfare regulations, under Title VI of ~he Civil Rights Act of 1964; for the ~kn~ns, Infants and ~ildren Program, FNS-64, Assurance of Cormpliance with the Depart~nt. of Agriculture Food and Nutrition Service, under Title VI of the Civil Rights Act of 1964; and ]{dS Form 641, Assurance of Compliance with Section 504 of the Rehabilitation Act of 1973. G. RESPfP~SIBILITIF~q OF THE STATE 1. The State shall provide to the Departamnt upon request technical assist~mce in the preparation of ~]~e Consolidated Contract/Activity Budgets and Contract Addenda. 2. Tile State shall specify %hose administrative forms/reports and their respective revision dates that are required by partic~]lar activities pertaining to %he Department's budget with the State in Listin~ of Required Fiscal .and .Statistical ~ (Addendum l) for the contract period. Ne~ forms/reports not listed in Required Fiscal and Statistical Re_~oorts shall be impl£~nented during a contract period only with the approval of ~he State Health Director. 3. 9he Skate shall provide to the Department within thirty (30) days after receiving an activity budget frcm the Department an approved signed copy of ~e budget.
