" <br /> <br />continuation, renewal, or extensions are subject to the <br />availability of funds for the purposes set forth in this <br />agreement. <br /> <br />(a) Reimbursement to be..Base~ on Performance. Payments shall <br /> be made to the Agency by the Council based on the <br /> Agency's performance in providing the services described <br /> in paragraph 2. Within five working days following the <br /> end of each calendar month of the contract period, the <br /> Agency shall submit to the Council a report showing the <br /> D~mher of service units provided for each service funded <br /> under this contract. <br /> <br /> Service units reported to the Council must be d~cumented <br /> by the Agency so that the following information can be <br /> verified: 1) service units are provided to an eligible <br /> individual, 2) service units are provided in accordance <br /> with the North Carolina Division of Aging Service <br /> Standards, and 3) service units are counted according to <br /> the appropriate service unit definitions which are <br /> included in the Service Standards referenced in paragraph <br /> 2. <br /> <br /> Subject to the Council's right to verify the accuracy cf <br /> the Agency's service unit report and the satisfactory <br /> performance of the work evidenced thereby, the Council' <br /> will make payments to the Agency. The amount of each <br /> monthly payment shall be calculated by multiplying the <br /> number of service units provided by the "AAA Unit Rate" <br /> shown in paragraph 2. <br /> <br />(b) ComDarison._o.f .the Budgeted ~A Unit Cost Rate to..~he <br /> Actual Cost. The Agency will also be required to submit <br /> a financial statement, in a form approved by the Council, <br /> as and when requested by the Council. The Agency's <br /> financial statement must be submitted within fifteen (15) <br /> days following the end of the particular accounting <br /> period, and it must document the Agency's accrued <br /> expenses and collected revenues by service so that the <br /> Council can compare the budgeted serVice unit cost to the <br /> actual service unit cost. In addition, the Agency shall <br /> submit a completed unit cost worksheet which reflects <br /> actual cost and revenue data for the period covered by <br /> this contract. This worksheet shall be submitted to the <br /> Council prior to December 15, 1990. <br /> <br />(c) Ad~ustmen~ to the_.Budqeted ~AA Unit cost. The Council <br /> will consider adjustments to the budgeted AAA unit rate <br /> after January 1~ 1990, upon the request of the Agency <br /> and if an analysis of actual cost and units provided <br /> through that point in the grant year shows: 1) the <br /> Agency's actual increase in expenses pertaining to this <br /> contract is due to extraordinarily higher costs which are <br /> beyond the control of the Agency,-or 2) there is a <br /> substantial difference between the number of service <br /> units planned and the number Of service units actually <br /> delivered° If the number of service units delivered is <br /> ."-.lower ~han planned, the Agency must make every effort to <br /> <br /> <br />
