7. The first responder ~nd the first responder agency shall be liable for their actions pertaining to the care of the patient prior to the arrival of the ambulance provider. SECTION g: Standards for Record Keeping. I. Each first responder agency shall maintain the following records: A. Record of Patient Treatment: For each patient rendered assistance by the first responder agency, the roi lowing Information will be recorded on a patient treatment form: (1) Name, age, and address of the patient. (2) Date ~nd time treatment rendered. (3) Location of treatment rendered. (4) P~t ient complaints and condition at the time of treatment rendered. (3) P~tient vital signs and time recorded. (6) Treatment rendered the patient by the first responder. (7) Name of the member of the ambulance provider to whom patient Is relinquished. (8) Printed names and signatures of the 'first respon~ers who provided patient care. B. A preliminary copy of t~e record of P~tient Treatment shall be given to the ambulance crew when he/she assumes control of the patient's care at the emergency scene. C_ A copy of the completed Record of Patient Treatment will be given to the Director of Cabarrus County Emergency Services at any time prior to the quarterly meeting of the Cabarrus County EMS Advisory Soard. This copy will be used for audit and D. The First Responder report will contain the dispatch number. The number will be the same as the EMS dispatch number. SECTION 10: Grievance Procedure. I.First responder agency will be responsible for resolving gr!evances between- Its own members. 2. Grievances from a first responder agency toward Cabarrus County Ambulance Service shall be submitted in writing by the first responder and co-signed by t~e Fire Chief or his/her designee to the director of Cabarrus County Emergency Services.
