A copy of a suggested form for the questions to be used <br /> at the referendum and in the ballot booklets for hand- <br /> ing to the printer who will prepare such items and an <br /> extra copy of such form to enable you to proofread the <br /> text thereof prior to printing. <br /> <br /> After the enclosed proceedings have been taken, please <br />provide me with two certified copies thereof and two certified <br />copies of the sworn statement of debt. After the bond orders <br />have been published, please provide me with two publisher's affi- <br />davits, with printed Clippings attached, to evidence each such <br />publication.
