MODEL DIABETES/GLAUCO~,~ PROJEC7 SUMMARY <br /> <br /> The model Diabetes/Glaucoma Projecl is deslgn~d to meet a well documented <br />need in Cabarrus County. The goals of the project are multifaceted In order <br />to Identify persons at risk and educate them as to lifestyle changes which can <br />be made to control their condition. The prlnclplegoals of the project are <br />to e~ucate the general public concerning diabetes and glaucoma and to provide <br />screening for citizens of Cabarrus ~ounty over 35 years of age. Through these <br />steps, the prevention of blindness and a reduction of health care costs through <br />the prevention of hospitalization will be acccmpllshed. <br /> <br /> The keys.to prevention are early detection and appropriate educatlon. The <br />projegt will be coordinated with local opthamologists, optometrists, Diabetes <br />Association, Lions Clubs, and other health department services. <br /> <br /> T~ese goals will be accompllshed through the efforts of existing staff <br /> <br />with the addition of a public health nurse. The responsibilities of this project <br />nurse Will Include Interviewing of screenees, testing of visual acuity and <br />performance of.tonometry testing. In addition, she w~ll'be responsible for <br />all teaching, follow-up of positive screenees and records. A register will be <br />maintained of all cases discovered. Finally, the project nurse will coordinate <br />the activities of community groups dealing with these two diseases. <br /> <br /> Evaluation of project will De two-fold, A determination of cost effectiveness <br />wil be made based on number of patients seen. The educational effectiveness <br />wil be determined through a pre/post test. <br /> <br />BUDGET <br /> <br /> Salaries - PHN II $14.185.00 <br /> Fringes 2,128.00 <br /> Supplies 2,687]00 <br /> Travel of PHN 3~000.00 <br /> TOTAL $22,000.00 <br /> <br /> <br />
