January 20, 2015 (Regular Meeting) Page 2255 <br />• Report to the court <br />(3) Annual Pre - Placement Assessment Update <br />• At least one home visit for data collection and a written addendum <br />to the original adoption assessment describing significant changes. <br />No fee shall be charged except pursuant to a written fee agreement which must <br />be signed by the parties to be charged prior to the beginning of the <br />preparation. The fee agreement shall not be based on the outcome of the <br />report or the adoption proceeding. All fees shall be paid prior to any <br />assessments or reports to the Court. Fees are to be paid to the Cabarrus <br />County DHS during regular business hours and receipts will be issued. Fees <br />are non - refundable upon the initiation of service delivery. <br />Fee Structure <br />(1) No fee will be charged when the following circumstance exists: <br />• The family has identified an adoptee who is or was in the custody <br />and placement responsibility of the Department of Social Services, <br />and the adoptive family continues to pursue the adoption of the <br />identified child <br />(2) � of the established fee will be charged to families under 200% of the <br />federal Poverty level. <br />(3) For all other families, the standard fees will apply: <br />Stepparent and Relative Adoptions (PPA and CR) <br />Independent adoptions (PPA and CR) <br />Annual Pre - Placement Assessment Update <br />Pre - Placement Update and Monitoring <br />Pre - Placement Agreement <br />Court Report only <br />Independent Home Study <br />Intermediary Services <br />$1,500.00 <br />$1,500.00 <br />$400.00 <br />$800.00 <br />$1,200.00 <br />$300.00 for first child <br />and additional $100 for <br />each subsequent child <br />$800.00 <br />$250.00 initial fee up <br />to 5.5 hours <br />$45.00 each additional <br />hour <br />Cabarrus County DHS retains the sole discretion of reducing or waiving any <br />fee if circumstances exist that would prevent a family from paying such fees <br />and delaying the adoption service would cause undue hardship on the well- <br />being of the child in question. <br />F - DHS - Community Alternatives program Divestment <br />The Community Alternatives Program (CAP) is a Medicaid based program <br />with the goal of helping nursing home level of care citizens remain at home <br />through a comprehensive service plan. The goal of the program is to promote <br />a person being able to remain in their home safely and defer the costs of <br />nursing home placement through intensive in -home services_ <br />The Cabarrus County Department of Human Services is the lead agency for <br />CAP services for the county and has been administering this program since its <br />inception. The Department has historically operated the program as a budget <br />neutral program, where revenues at least met expenses. Three years ago, the <br />Department divested of the CAP -C program (CAP services for children) in an <br />effort to reduce expenditures due to decreasing revenues. Over the past two <br />years, additional Medicaid reimbursement reductions have resulted in the <br />program expenses exceeding available revenue. In addition, the conversion to <br />the North Carolina Tracks billing system for Medicaid has created an <br />administrative burden for billing CAP services, resulting in delayed and lost <br />revenues. <br />With these current trends, the Department is recommending divesting <br />itself of the CAP program. The primary factors for this recommendation <br />include: <br />- Lowered reimbursement levels creating challenges to operating <br />program as a budget neutral service, <br />- Rising costs of providing services, <br />
