135 <br /> <br />o <br /> <br />No agency or department shall furnish, operate, conduct, maintain, <br />advertise, be engaged in or profess to be engaged in service as a <br />first responder agency unless that agency holds a currently valid <br />certificate to operate as a first responder issued by the Cabarrus <br />County Office of Emergency Medical Services and approved by the <br />Cabarrus County Board of Commissioners or its designated <br />representative. <br />No person shall operate, conduct, maintain, advertise be <br />engaged in or profess to be engaged in business or service of a <br />first responder unless that person is affiliated with a certified <br />first responder agency and holds a currently valid certificate to <br />operate as a first responder issued by the Cabarrus County Office of <br />Emergency Medical Services and approved by the agencyfs Fire Chief <br />or designee. <br /> <br />SECTION 3: Application and Granting Process for Certification as a First <br /> Responder or an Individual First Responder. <br /> <br />Application for certificate to operate as a first responder agency <br />in Cabarrus County by an applicant agency shall be filed with the <br />Cabarrus County Office of Emergency Medical Services. This <br />application shall be made upon such forms as may be prepared by the <br />Cabarrus County Office of Emergency Medical Services and shall <br />contain: <br /> <br />o <br /> <br />o <br /> <br />A. The name and address of the applicant agency. <br />B. The fire district in which it will operate. <br /> A plan describing how the agency will respond when dispatched <br /> on first responder calls. <br />D. Any information Cabarrus County Office of Emergency Medical <br /> Services shall deem reasonably necessary for a fair <br /> determination of the capacity of the applicant agency to <br /> provide first responder service in the service area in <br /> accordance with Cabarrus County requirements. <br />Upon receipt of an application approved by the Advisory Board, a <br />certificate to operate as a first responder agency in Cabarrus <br />County, the Cabarrus County office of the Emergency Medical Services <br />will forward such applications to the Cabarrus County Board of <br />Commissioners for their approval. <br />The Cabarrus County Board of Commissioners shall approve or <br />diSapprove all applications for first responder agency status and <br />grant certificates to operate as a first responder agency in <br />Cabarrus County. <br />Application for certificate to function as an individual first <br />responder in Cabarrus County by an applicant shall be filed with the <br />Cabarrus County Office of Emergency Medical Services by the <br />applicant Fire Chief or his/her designee with proper approval from <br />the corporate Board of Directors. This application shall be made <br />upon such forms as may be prepared by Cabarrus County Emergency <br />Medical Services and shall contain: <br /> <br />A. The name, age, social security number, driver's <br /> license number, and address of the applicant individual. <br />B. The dates, location, and instructor of a successfully <br /> completed basic first responder training course by the <br /> applicant individual. <br />C. The name of the first responder agency with which the <br /> applicant individual is to be affiliated. <br />Upon receipt of application for certificate to operate as an <br />individual first responder in Cabarrus County, the Director of <br />Emergency Medical Services or his designated representative will <br />approve or disapprove all applications based on criteria in Section <br />5 of this document. <br />Upon approval to operate as an individual first responder in <br />Cabarrus County, a certificate and pocket card will be issued by <br />Cabarrus County Office of Emergency Medical Services. <br /> <br /> <br />
