288

C. Provide CTS with information concerning authorized riders and pickups
between the hours of 9:00 a.m. and 1:00 p.m. on the day prior to the day service
is requested;

D. When requesting vans for special trips outside the regular system operation
will make their request to CTS with sufficient notice to allow CTS to arrange
for the vehicle(s) and driver(s);

E. Reimburse CTS for transportation provided at the rate of ninety three cents
($.93) per vehicle mile plus three dollars and fifty cents ($3.50) surcharge per
bus hour of operation when CTS drivers are provided, with the following
conditions attached:

1. Increases in the average per-gallon price of gasoline paid each
month over the average per-gallon price of $.72, will be compensated by
an increase in the mileage rate paid to CTS as follows: for every five
cents ($0.05) increase per gallon of gasoline, the mileage reimbursement
rate will be increased by one-half ($.005) cents per vehicle mile; the same
condition applies for decreases in the average per-gallon price of
gasoline;

2. In the event that an agency requests CTS to provide transportation
service for its clients to a destination that would require CTS's driver(s)
to be (1) inactive for a period longer than one-quarter hour (15 minutes)
and/or (2) stay overnight with the clients at the destination, then the
approved rate per mile will be supplemented as follows:

EXAMPLE 1 - Trips of up to one day duration requiring a driver and
vehicle to be out of service for normal operations (to the nearest
quarter-hour), then CTS will be compensated for the balance at a rate of
$10.00 per hour per driver;

EXAMPLE 2 - Overnight trips of two days duration: for the first day,
CTS will be compensated at a rate of $10.00 per hour per driver for the
difference (if any) between the time of arrival at the destination and
5:00 p.m., and on the second day for the difference (if any) between 8:00
a.m. and the time of departure. Lodging and meal expenses incurred by
CTS's driver(s) will be reimbursed at cost;

EXAMPLE 3 - Overnight trips of three or more days: for the first day
and last days, the terms of Example 2'will apply; and for each additional
day, the terms of example 1 will apply. Lodging and meal expenses incurred
by CTS's driver(s) will be reimbursed at cost of the entire period;

EXAMPLE 4 - Wait time will be charged any agency where the vehicle
and driver must remain on site more than fifteen (15) minutes. Wait-time
will be at a rate of ten ($10.00) dollars per hour with a one hour minimum.

3. For every ten ($0.10) cents increase in the Federal Minimum Wage
Rate (FMWR), the mileage reimbursement rate will be increased by one cent
($0.01) per mile;

4. Increases or decreases of annual insurance premiums shall be adjusted
and prorated as follows: when during the contract period the insurance
premiums increase/decrease more than five percent (5%) from the current
rates the charge per vehicle mile shall be adjusted to reflect an
increase/decrease in premiums. Increases/decreases in premiums up to and
including five percent (5%) variance will not effect the rate;

5. The County will pay CTS for transportation services within 10 days
after the receipt of each monthly billing;

6. The rate per mile above will be based on the assumption of 350,000
miles of operation. If the average monthly service mileage consistently
falls below or rises above 1/12 of 350,000, the County agrees to
renegotiate the rate;

7. The County will reimburse CTS at an additional rate to be added to
the rates herein contained for vehicles provided by CTS at the County's
